If registered rough diamond traders would like to amend any of the particulars in the registration, please click here (pdf format) to download the form for Application for Amendment of Registration Particulars, or obtain the form from the Rough Diamonds and Ozone Depleting Substances Licensing Unit at Rm 1604, 16/F, Trade and Industry Tower, 3 Concorde Road, Kowloon City, Hong Kong.

Duly completed form together with relevant supporting documents, e.g. copies of Certificate of Change of Name (CR), or valid Business Registration Certificate etc. should be lodged to the Rough Diamonds and Ozone Depleting Substances Licensing Unit by hand, by mail, or by electronic submission.
