| In case a registered stockholder/approved rice storage place has been
denied access to its account due to repeated login failures or has forgotten its User Name/Password,
the business concerned has to notify the Department by filling in the Form
and return it to the Rice Control Unit (Fax No. : 3525 0987) or lodge the form electronically via the Department’s e-Form Submission Service.
