Certificate of Origin - Form A
Procedures and Supporting Documents
A completed, typewritten Certificate of Origin Form A (pdf format) in triplicate and an application form TIC 185B (Rev.) (pdf format) in duplicate should be handed in to CO/Form A Section of Factory Registration and Origin Certification Branch on the 2nd floor of Trade and Industry Department Tower, 700 Nathan Road, Kowloon. The application should be made at least two clear working days before departure of the consignment. Blank forms of Certificates of Origin Form A are available for sale at the following stationers:
- Ying Wah Offset Printing Press (Enquiry: 2401 1080)
- Swindon Book Co. Ltd. (Enquiry: 2366 8001)
- Cheung Lee Printing Co. (Enquiry: 2543 8069)
The application form TIC 185B (Rev.) can be purchased at the Collection and Form Sales Office at 1/F of the Trade and Industry Department Tower.
Request for amendment or cancellation of an issued Certificate of Origin Form A needs to be supported by an application for amendment and cancellation of Certificate of Origin Form A . The application form is also available free of charge from CO/Form A Section located at 2/F, Trade and Industry Department Tower.
