By appending the electronic signature of the carrier/forwarder to this notification, I, for and on behalf of the carrier/forwarder of this notification, hereby make the following declarations: -

1. I declare that the transhipment cargo covered by this notification will be handled by me whilst in Hong Kong, and that I have read and understood, and agree to abide by the conditions of exemption as set out in the Textiles Trader Registration (TTR) Circular No. 6/98 'Textiles Trader Registration Scheme : Conditions of Exemption' and any subsequent updates issued by the Director-General of Trade and Industry (DGTI);

I further declare that I have completed this notification according to the specified requirements for completing this type of notification as set out in the TTR Circular No. 12/2003 'Introduction of Electronic Service for Textiles Notifications' and any subsequent updates issued by DGTI, and that the information and particulars given herein are true and correct; and
   
   

3. I further declare that the physical custody of the transhipment cargo will remain with me at all times while it is in Hong Kong and that no processing or substitution of the transhipment cargo will take place in Hong Kong.

By appending the electronic signature of the exporter to this notification, I, for and on behalf of the exporter of this notification, hereby make the following declarations:-

1. I declare that the goods covered by this notification are bona fide samples of no commercial value and are not for sale in the importing country;
   
   

2. I further declare that the goods are of Hong Kong origin in accordance with the specifications laid down by the Director-General of Trade and Industry in the Certificate of Origin Circular No. 07/02 'Hong Kong Origin Rules' and any subsequent updates; and
   
   

3. I further declare that the goods covered by this notification are not packed and invoiced together with goods not covered by this notification.

By appending the electronic signature of the exporter to this notification, I, for and on behalf of the exporter of this notification, hereby declare that in respect of the goods covered by this notification, no principal manufacturing process has been carried out in Hong Kong on the goods above which would permit the issue of a Certificate of Hong Kong Origin in accordance with the specifications laid down by the Director-General of Trade and Industry.

By appending the electronic signature of the exporter to this notification, I, for and on behalf of the exporter of this notification, hereby make the following declarations:-

1. I declare that the goods covered by this notification are bona fide and marked samples not exceeding US$800 in free-on-board (f.o.b.) value for export to the USA and not for resale;
   
   

2. I further declare that the goods are of Hong Kong origin under the certification of origin or export licensing system administered by the Director-General of Trade and Industry (DGTI) and are manufactured by the manufacturer(s) as stated in this notification;
   
   

3. I further declare that the goods have been stamped or labelled in accordance with the marking requirements for exports to the USA of properly marked textiles samples of Hong Kong origin not exceeding US$800 in f.o.b. value as stipulated in the Textiles Trader Registration Circulars No. 6/98 'Textiles Trader Registration Scheme : Conditions of Exemption' and No. 12/2003 'Introduction of Electronic Service for Textiles Notifications', and any subsequent updates issued by the DGTI, and that a declaration "Marked Sample - Not for Resale" has been made in the invoice covering the goods; and
   
   

4. I further declare that the goods are packed and invoiced separately from other goods not covered by this notification, and that I have not exported any other marked commercial textiles sample(s) to the same consignee in or on the same vessel, aircraft or vehicle.

By appending the electronic signature of the exporter to this notification, I, for and on behalf of the exporter of this notification, hereby make the following declarations:-

1. I declare that the goods covered by this notification are bona fide and stamped/mutilated samples having no commercial value for export to the USA and not for resale;
   
   

2. I further declare that the goods are of Hong Kong origin under the certification of origin or export licensing system administered by the Director-General of Trade and Industry (DGTI) and are manufactured by the manufacturer(s) as stated in this notification; and
   

3. I further declare that the goods have been mutilated or stamped in accordance with the requirements for exports to the USA of mutilated or stamped textiles samples of Hong Kong origin having no commercial value as stipulated in the Textiles Trader Registration Circulars No. 6/98 'Textiles Trader Registration Scheme : Conditions of Exemption' and No. 12/2003 'Introduction of Electronic Service for Textiles Notifications', and any subsequent updates issued by the DGTI, and that a declaration "Stamped or Mutilated Sample - Not for Resale" has been made in the invoice covering the goods.

By appending the electronic signature of the exporter to this notification, I, for and on behalf of the exporter of this notification, hereby declare that the goods covered by this notification are piece-knitted garments, and that knitting of the knit-to-shape panels of these piece-knitted garments has been done outside Hong Kong.

By appending the electronic signature of the exporter to this notification, I, for and on behalf of the exporter, hereby declare that the goods covered by this notification are returned cargo.
