appending the electronic signature of the manufacturer to this Production
Notification, I hereby declare that the information given herein
has been checked by me and is true, that the cut pieces/component
parts for making up of the finished goods declared herein are those
specified by the Director-General of Trade, that the finished goods
shall be manufactured in the manner as declared in this Production
Notification, and that the assembly work to transform the cut pieces/component
parts into the finished goods shall be done in the manufacturer's/subcontractor's
premises or place in Hong Kong registered with Trade Department
as represented by its/their address code(s) declared in this Production
Notification and by outworker(s) in Hong Kong. I further declare
that I have read and am conversant with the contents of the Certificate
of Origin Circulars and all the Certification Branch Circulars currently
in force and have read and understood, and agree to abide by the
conditions of Production Notification issued by the Director-General
of Trade. I also authorize the Director-General of Trade to disclose
all or any of the information provided in this Production Notification
to any third parties in Hong Kong or elsewhere.

appending the electronic signature of the subcontractor to this
Production Notification, I hereby declare that the information given
herein has been checked by me and is true, that the cut pieces/component
parts for making up of the finished goods declared herein are those
specified by the Director-General of Trade and that the assembly
work to transform the cut pieces/component parts into finished goods
in respect of the subcontractor quantity shall be done in my factory
in Hong Kong registered with Trade Department as represented by
its address code declared in this Production Notification. I further
declare that I have read and am conversant with the contents of
the Certificate of Origin Circulars and all the Certification Branch
Circulars currently in force and have read and understood, and agree
to abide by the conditions of Production Notification issued by
the Director-General of Trade. I also authorize the Director-General
of Trade to disclose all or any of the information provided in this
Production Notification to any third parties in Hong Kong or elsewhere.
